- COPY OF VALID DHS REGISTRATION CERTIFICATE OF THE HOSPITAL/NURSING HOME.
- AUTHORIZATION LETTER IN THE NAME OF DESIGNATED MEDICAL PRACTITIONER AS OVER-ALL INCHARGE. COPY OF REGISTRATION CERTIFICATE OF THE DOCTOR OVER-ALL INCHARGE.
- COPY OF REGISTRATION CERTIFICATE OF THE DOCTOR OVER-ALL INCHARGE.
- PROOF OF CONSTITUTION OF THE HOSPITAL.
- NAME OF THE QUALIFIED MEDICAL PRACTITIONER(S) WHO WOULD PRESCRIBE ENDs.
DOCUMENTS TO BE SUBMITTED EVERY YEAR BY THE RMI
- FORM 3I-ANNUAL RETURN OF PROCUREMENT/DISBURSEMENT OF ESSENTIAL NARCOTIC DRUGS (TO BE FILLED BY RMI).
- FORM 3J- ESTIMATE OF ANNUAL REQUIREMENT OF ESSENTIAL NARCOTIC DRUGS.
- COPY OF VALID DHS REGISTRATION CERTIFICATE OF THE HOSPITAL/NURSING HOME.
- DOCUMENTS IN CASE OF CHANGES REGARDING DOCTOR INCHARGE/CONSTITUTION OF THE RMI.
- NAME OF THE QUALIFIED MEDICAL PRACTITIONER(S) WHO WOULD PRESCRIBE ENDs.
